After Your Stay & Medical Records

Discharge from the hospital to home requires the successful transfer of information from provider to the patient and family to reduce adverse events and prevent readmissions.  Engaging patients and families in the discharge planning process helps make this transition in care safe and effective.

Medication

Please pay close attention to your medication instructions upon discharge.  You will be sent home with paper instructions, but please be sure to ask questions to confirm your understanding of your medication schedule.  

 

Medical Records: Accessing Your Medical Record Online

If you would like to view your medical record online, please access Barrett Hospital & HealthCare’s MyChart® electronic records system

Access to your electronic medical record is available through Barrett Hospital & HealthCare’s patient portal, known as MyChart. MyChart is an online tool offered by most Epic healthcare organizations that gives patients secure access to the same medical records your provider uses and provides convenient self-service functions that reduce costs and increase patient satisfaction. With MyChart, you can view your medical history and test results, message your healthcare provider, manage your appointments and request prescription refills -- all from your smartphone or computer.

To register for MyChart, contact your health care team for a “MyChart activation code.” 

You may also visit our Medical Records Team at the Hospital, Monday - Friday from 8 a.m. - 5 p.m. and they will assist you throughout the process. Medical Records can be reached at 406-683-3073 for the Hospital and 406-683-1179 for the Clinic.

 

Medical Records

Obtaining a Copy of Your Medical Record in Person.  If you are over the age of 18, you may obtain a copy of your medical record by calling the Medical Records department at 406-683-3073.  Identification must be provided when records are picked up.  A parent's or legal guardian's signature is required to obtain medical records for any patient under the age of 18.
