Be ready by packing the following:

Your health insurance plan card

Personal identification (valid driver's license, state identification card or passport)

All medications you are taking in their original container

Essential personal items, such as glasses, dentures, and hearing aids

Personal hygiene items — comb/brush, deodorant, toothpaste, toothbrush and other toiletries you may need

Clothing for your discharge

Reading/writing materials and other personal items you'll want during your stay

Cell phone or tablet and charger

It is also good to have an advance directive in place and to let your loved ones and Barrett Hospital & HealthCare staff know how to access it if need be. A Five Wishes® advance directive ensures that your family and healthcare team know your personal healthcare wishes and goals. Forms are available at the Hospital Medical Records Department and at our Clinics.
